Immunization Information

Submit Your Immunization Records:

All students at Emory University are required to abide by the immunization guidelines established by the Centers for Disease Control and the American College Health Association. You can learn more about which specific immunizations are required by visiting the Student Health Services website.

Prior to the first day of classes, you must complete the Immunization Form, which is available through the Emory Healthcare Patient Portal. Failure to complete and fulfill immunization requirements will result in an inability to register for class.

**Please Note **

Immunizations can now be uploaded via your Patient Portal.
The document should be in a PDF or JPEG format (although a PDF format is preferred). A Microsoft Word version will not be accepted.
Please only utilize one method to provide immunization records and refrain from using multiple methods to provide your records. (I.e. Uploading and emailing your records).
Once your records have been uploaded, please note that they will not automatically be reflected as it takes several days to verify your records. It may take Student Health Services several business days to verify and update your records and send the update to our office.
